On-Going Care Management
The RN will continue to over-see the client’s care, which includes:
- Scheduled in-home visits
- Supervise in-home personnel and services
- Monitor health status, reporting changes to family and physicians
- Monitor and assist with medication administration and prescription refills
- Coordinate medical appointments and tests
- Schedule transportation with in-home personnel
- Report concerns and emergencies to families
- A liaison between physicians, families and clients-ensure Continuity of Care and the Right Care by the Right Provider
- Ensure physicians’ plan of treatment is followed
- Make referrals to appropriate providers
- Suggest appropriate services such as home health, physical therapy, hospice, etc..
- Provide education to assist clients and families in making informed decision related to their healthcare
- Implement an agreed-upon, strength-based Plan of Care created by the CSOA Interdisciplinary Team of Providers
- Goal: to achieve the highest quality of life in the least restrictive environment, promoting health and improving well-being
- Family Meetings upon request
