During the Initial Consult the RN, client and family will:

• Discuss the concerns regarding the current situation
• Assess the medical-needs of the client related to the current circumstances
• Discuss the short and long-term goals everyone would like to achieve
• Comprehensive brain-storming with client and family to begin development an Individualized Plan of Care
