Individual Plan of Care
CSOA’s RN will develop an Individualized Plan of Care, which includes:
- Implement in-home services to assist with every day needs
- Detailed Plan of Care related to each diagnosis
- Coordinate with family a Medication Schedule
- Combine the current care with additional specialized services to create an Individualized Plan of Treatment in tandem with the physicians’ Treatment Plan
- Referrals to appropriate resources and providers
- Goal: create an agreed-upon, strength-based Plan of Care created by the CSOA Interdisciplinary Team of Providers
