Initial Consult:

Discuss concerns regarding current situation
Assess current circumstances
Discuss short and long-term options

Comprehensive Assessment:

Assess needs, abilities, strengths
Discuss medical history
Identify problems and present solutions
Consult with family members and providers
Evaluate current care providers and services
Recommend any additional services needed to improve safety or quality of life
Review insurance, medical, financial matters, eligibility for assistance and community services

Individualized Plan of Care: included with Comprehensive Assessment

Goal: to achieve the highest quality of life in the least restrictive setting, promoting health and improving well-being
Referrals to appropriate resources and providers
If necessary, access possible funding sources for health care needs

Implementation of an agreed upon, strengths-based Plan of Care created by the CSOA interdisciplinary team of providers

On-Going Care Management:

Liaison with healthcare providers to interpret results/care
Interact with personal physician to support overall treatment plan
Liaison for families at a distance
Monitor health-status
Provide education to assist clients and families in making informed decisions
Locate and manage hired home care/caregivers
Ongoing re-evaluation/assessment of client needs and quality of services provided by all care providers
Family Meetings upon request

Crisis Intervention:

24/7 on-call assistance when emergencies occur
Accompany client to hospital
